I looked at your performance appraisal for executive unfortunately you have not touched on executive qualities, such as leadership, managing multiple tasks, problem solving, department turnover
Although you have asked for acheivement in your form but they dont seem to be planned, `
There is no rating sacle in your form
Since it has no rating system then it is not tied to any increment or compensation nor bonus
it has no proper training & development plan
Your evaluation should be quarterly since its for executives
It should reflect how large is the team your managing
The competencies there are evaluated against should be more specific not just a yes & No
